Provenance: this copy is warmly inscribed and signed by the author to fellow neurosurgeon Sir Hugh William Bell Cairns (1896-1952). It also carries the later ownership signature, date of acquisition (11 December 1956), and bookplate of Professor Donald Simpson AO (1927-2018), another eminent Australian neurosurgeon.
